Implementing FY2019 dependent status verification

We request the cooperation of subject persons for the FY2019 dependent status verification. This verification will be carried out as described below.

 

Details

 

1. Background of dependent status verification

Since the relationship between insured persons and dependents certified by respective health insurance societies can change over time, the Ministry of Health, Labour and Welfare of Japan has instructed health insurance societies to verify the status of dependents annually.

 

2. Subject persons

Insured persons with dependents

*Not including Voluntarily and Continuously Insured Persons, those working overseas accompanied by family members, and those planning to resign or retire by the end of August 2019

*Changes from last year:

・ Last year: You were asked to submit documents for all dependents.

・ This year: Please submit documents only for those earning annual income within the range from 1.03 million yen to the base amount (1.3 million or 1.8 million yen, depending on age and other factors). However, we ask that you submit documents for all dependents who have self-employment income.

 

4. Verification period

Monday, July 29 – Saturday, August 31, 2019

 

5. Verification method and schedule

① The request will be sent to each subject person by email (sent from: genkyo_chosa@takeda.co.jp).

1. Advance notification concerning the verification will be sent by email on Monday, July 22.

2. The URL for the web system for dependent status verification and the user ID and password required to log in to the system will be sent by email on Monday, July 29.

② Click the system URL to access the web system. Answer the questions and provide other information as instructed.

*You can answer questions and upload documents (files) from a computer or smartphone.

*Depending on dependent status and other factors, you may be asked to submit documents. Do so by uploading the documents as computer files (valid formats: pdf, jpg, or png).

>> Data will be encrypted before transmission.

>> Please retain the original documents until the verification process is complete (at the end of September). The Society may make inquiries about submitted documents until that date.

*No documents need to be submitted for dependents who are students in high school or lower grades or dependents who were certified as dependents less than one year ago. However, please update registered addresses and other data if this information has changed.
