Documents to submit when applying for dependent certification

The following documents must be submitted in addition to the Application Form for Dependent Certification.
You may be asked to attach and submit documentation in addition to the documents indicated below.
Note that the Society may check with related institutions to confirm the documents attached.

- Applying to name a child as a dependent (newborn, student)
- Applying to name a child as a dependent (e.g., certification after leaving employment)
- Handling of self-employed family members (including agricultural workers)
- Applying to name a spouse as a dependent
- Applying to name an actual (adopted) parent or a mother or father in law with you as a dependent
- ** Submit documents attesting to kinship (for example, an official partial copy of a family register) and the following documents if the family member resides overseas and does not have a certificate of residence indicating a residence in Japan.
