2023年度の被扶養者現況調査を次のとおり実施いたします。
本調査は、被扶養者様が引き続き健康保険の扶養基準を満たしていることを確認するための法定の調査です。
調査対象の皆様には武田健保より依頼メールをお送りいたします。メールが届いた場合には、お手数をおかけいたしますが、調査へのご協力をお願いいたします。
なお、回答がない場合、法令により、該当のご家族様の被保険者証を無効とし、保険給付を行わない場合がありますので、ご注意ください。
１．調査対象
健保組合が調査の必要があると判断したご家族
【調査対象のご家族の例】
① 2022年の給与収入・年金収入が一定以上あると思われるご家族
② 事業所得または不動産所得を申告（赤字を含む）しているご家族
③ 配偶者や子供以外の別居家族
※健保組合は国の施策により、マイナンバーによる情報照会を利用して課税情報を確認することが認められていますので、課税情報を基に対象者を選定します。
２．調査方法
Webシステム（MY HEALTH WEB）にて回答・書類の提出をお願いいたします。なお、本調査業務は、（株）法研に委託します。
（１）対象の被保険者様へ８月31日に健保組合から調査依頼メールを発信します。

（２）調査開始日に、（株）法研より、対象の被保険者宛にMY HEALTH WEBを使った調査方法についてメールにて連絡があります。
（発信者アドレス：takeda-kenpo@houken.ne.jp）
（３）調査依頼メールを受け取られた方は、MY HEALTH WEBにログインし、回答・書類の提出をお願いします。

We will conduct FY 2023 dependent eligibility survey as follows.
This statutory survey is conducted to confirm that your dependents still meet the health insurance program’s dependent criteria.
Takeda Health Insurance Society will send a request e-mail to insureds subject to this survey. You are kindly requested to respond to this survey if you receive this e-mail.
Please note that a failure to respond can result in invalidation of the health insurance for your relevant family member(s) and no payment of insurance benefits in accordance with the regulation in Japan.
1. Those subject to this survey
Family members considered by the Society as subjects of this survey

a. Those deemed to have earned a certain level of salary and/or pension income in 2022
b. Those who declared a business income and/or real estate income (including those in deficit)
c. Those living separately other than spouse
* The Health Insurance Society is granted by the national government access to insureds’ taxation information using their social security numbers, based on which we selected subjects of this survey.
2. Survey method
Please use the online system (MY HEALTH WEB) to respond and submit necessary documents. This survey is outsourced to Hoken Co.,Ltd.
(1) Insureds subject to this survey will receive a request e-mail from the Takeda Health Insurance Society around August 31.

(2) On the start date of the survey, Hoken Co.,Ltd. will send an e-mail explaining how to respond to this survey on MY HEALTH WEB to subject insureds.

(3) Please log in MY HEALTH WEB to respond and submit necessary documents if you receive the request.
