Marriage/divorce (change of name or address)

Apply to the Health Insurance Society if the name of the insured person or a dependent has changed or you want to add a new family member as a dependent due to marriage or other reason.

Change of name

To apply, provide the information specified on the following document and attach the health insurance card. Enclose a self-addressed return envelope if you are a Takeda Pharmaceutical Company Limited employee or a Voluntarily and Continuously Insured Person, and submit to the Health Insurance Society. If you are an employee of an establishment other than Takeda Pharmaceutical Company Limited, submit to person in charge of health insurance administration at your employer.

Required documents:
  • Notice of Change to Registered Health Insurance Information
Health insurance card before change
Deadline: Immediately after the name change
Applies to: Insured persons and dependents whose names have changed
Address inquiries to: Health Insurance Society
Notes:
  • ** In principle, complete the procedures above for the Health Insurance Society within five days if the name of an insured person or dependent has changed due to marriage, divorce, or other reason or you find an error in the registered data, such as the date of birth or name.

Change of address

<Insured persons>** Apply in the same way as for family members.

  • Takeda Pharmaceutical employees: Apply from Workday.
  • Employees of other companies: Contact the person in charge of health insurance administration at your company.
