期日を過ぎての申請は受付しませんので、ご留意ください。
申請期間：２０２４年４月５日(金)～４月３０日(火)
※2023年4月1日～2024年3月31日の利用分が対象となります

＜2023.3～お知らせ済みの内容＞
スポーツクラブの費用補助の変更について（2023年4月1日より）
武田健保では加入者を対象に、これまでコナミスポーツおよびルネサンスのみの都度利用者に対して利用料の補助をしておりましたが、２０２３年度よりご自身で選んだスポーツクラブの利用を補助する運用に変更しますので、下記のとおりお知らせします。
なお、費用補助を受けていただくためには、領収書が必須となりますので、利用された際の領収書は保管をお願いします。
１．対象者
被保険者および被扶養者である配偶者
２．対象となる施設
・トレーニングジム、室内プール、スタジオなどの運動施設を有し、会員に提供する
事業所（スポーツクラブ）が運営する施設のうち、対象者が自ら選定したもの
※プールのみ、テニスコートのみ等の事業所は対象外
３．費用補助額
1名あたり、次の金額を限度とする実費（消費税含む）。
（１）加入費用：年間 10,000円（1年あたり上限1回）
（２）利用費用：年間 12,000円
４．対象期間
毎年度４月１日～３月３１日
５． 申請方法
（１）申請時期：利用年度の翌年度４月５日～４月３０日
※ ２０２３年度利用分の申請は、２０２４年４月５日～３０日を予定。
（２）申請方法：申請システムから申請
※ 申請方法の詳細は改めてお知らせします。
【領収書についての注意点】
－申請される場合、領収書を必ず保管ください。
※領収書がない場合は費用補助を受ける事ができません。
※通帳の写しなどは領収書の代替になりません。
但し、領収書の発行が一切行われない施設を利用される場合は、よくある質問(FAQ)をご確認ください。
－以下の項目がない領収書は不可となりますのでご留意ください。
●利用施設名、●利用者氏名、●日付、●金額、●支払内容(*)
（*）入会金、利用料であることがわかること
６．支払時期
・申請月の翌月以降に各自の給与支払口座へ振り込みます。
・任意継続の方は、各自の登録済銀行口座へ振り込みます。
７．その他の留意事項
既存の法人契約について
・コナミスポーツとの契約は2023年3月末で終了しますので、引き続き利用をされる方は、個人プラン等への契約変更をお願いします（3月中に手続きをすれば登録料は無料）。
※詳細は、ご本人または被扶養者が都度会員として登録いただいている被保険者に個別メールにてご案内します。
・ルネサンスとの契約は継続しますので、再登録等の手続きは必要ありませんが、都度利用補助（500円）はありません。

Changes of Gym Subsidy Program in FY2023
We are pleased to announce that the Takeda Health Insurance Society will be expanding gym choices on private gyms, which were limited to Konami Sports Club and Renaissance only, into your local gyms from FY2023.
Please make sure to show the receipt issued by your local gym to receive subsidies from us.
1. Eligible person
Insured individuals and their spouses (dependent family members)
2. Eligible in-network gyms
Private gyms classified as “fitness clubs” under the Japan Standard Industry Classification established by the Ministry of Internal Affairs and Communications.
3. Annual subsidy
The following is the maximum subsidy per person, based on the cost they’ve paid. (Tax included)
（１）Initial membership fee: 10,000 yen (only once a year)
(２）Total of monthly fees: 12,000 yen
4. Applicable period
between April 1 and March 31 of the following year
5. Application Method
（１) Subsidy application period: Between April and April 30 on the following year
※Subsidy application period on gym membership during FY2023 will be between April 5 and April 30, 2024.
（２）How to apply: visit the application system that will be announced later

－Be sure to keep the receipt(s) for the application.
※If you have lost the receipts, we are afraid, but you cannot be subsidized.
※Caution; your bank book records are not certified as the receipt(s) from the gym.
－Please make sure that the all items below are included in the receipt.
●Private gym name, ●full name of the gym user, ●Date of issue, ●Total cost paid, and ●statement indicating initial membership fee to join and/or monthly fee
6. Payment period
The subsidy will be paid with your salary in May and later .
For voluntary insureds, the subsidy will be paid to their designated bank account.
7. Important notes
A corporate gym deal with Konami
・We are afraid, but our corporate gym deal with Konami will be ended at the end of March, 2023．
Those who want to continue with Konami should change their corporate gym deal into private deal.
(There will not be any extra fee in changing into private deal during this March)
※The insureds themselves or whose spouses have registered as gym users will receive email notifications.
・Our deal with Renaissance will be continuing as in the past, but you will not get an additional subsidy (500 yen for each visit).
