FY2021 Dependent Eligibility Survey

Please be advised that an annual survey in FY2021 to review eligibility status of your dependents will take place as scheduled below, based on the guidance from Ministry of Health, Labour and Welfare of Japan. The aim of this survey is to reconfirm that your family members have continued enrollment eligibility for Takeda health insurance.

Two surveys are scheduled this year.

◆ Survey Ⅰ:Self-check  [Period: from July 14 to Aug 17]

 1. Respondents to this survey

  All insured persons with dependents

2. Method

(1) You will receive a notification email sent by Takeda Kenpo on July 15.

(2) You need to use the self-check spreadsheet attached to the email to reconfirm that your dependents have   continued enrollment eligibility for Takeda health insurance at this moment.

(3) If your family member does not satisfy continued enrollment eligibility for Takeda health insurance, remove him/her from the list of dependents.

 For Takeda employees: remove the family member from the list of dependents through COMPANY.

 For affiliate employees: submit “Notification of Loss of Eligibility as Health Insurance Dependents” for persons in question 

(4) If your family member satisfies continued enrollment eligibility for Takeda health insurance, no report to Takeda Kenpo is required. (You do not have to take any specific steps)

 

◆Survey Ⅱ: Report of your dependent’s annual income [Deadline: Aug 27]

1. Respondents to this survey

   Insured persons with dependents who are randomly selected by Takeda Kenpo

2. Method

(1) You will receive a notification email sent by Takeda Kenpo on July 20.

(2) You need to submit the required documents to recertify dependents for continued enrollment such as “proof of annual earnings” of the dependent’s income including his/her “withholding slip” or “Taxation (Non-Taxation) Certificate”, both for year of 2020, to Takeda Kenpo.

(3) If your family member does not satisfy continued enrollment eligibility for Takeda health insurance, remove him/her from the list of dependents.

For Takeda employees: remove the family member from the list of dependents through COMPANY.

For affiliate employees: submit “Notification of Loss of Eligibility as Health Insurance Dependents” for persons in question
