[Reminder] Our flu shot subsidy program is coming to close

Please note that our flu shot subsidy program for FY2023 in the community will be closed on December 31. If you have been vaccinated against flu but have not applied for the subsidy, please complete the application process as soon as possible by the due date. Please be advised that you can no longer apply for this subsidy after that date.

 

1.Eligibility criteria

 ・  Insured of TPC Health Insurance Society who do not use TPC’s on-site flu shot program

 ・  Dependent family member

  ※Some sites host an on-site flu shot program. Detailed information will be provided by respective sites.

2.  Subsidy eligibility period and application period

                 Between Sunday, October 1 and Sunday, December 31

            [Applications submitted after this period are not accepted, NO EXCEPTION]

 

3. Number of flu shots and cost to be subsidized

 ・  Age 13 and over: 1 shot

 ・  Dependent family member under 13: 2 shots

  ※Age is as of the day of the flu shot.

 ・  Max. 5,000 yen per shot (including tax) will be covered.

 

4.Application method and receipt

  ① How to apply

  ・Apply via online application system

   Enter required information in the system and upload the photo(s) of the receipt. (Please make sure that you keep the receipt until the subsidy is paid as we may contact you for further information about the receipt)

    ・How to access the application system

         We’ve changed from the previous method of logging in to KENPOS with your

   PC or smartphone to a new one of clicking the banner on our website (https://www.takeda-kenpo.or.jp/en/) or hitting the link below.

   ※The insured’s log in ID and password is required. Check them at your log in screen. (Have your health insurance card at hand)

  ②Receipt

  [Note] Each receipt must include recipient’s full name, date of shot, actual cost of shot and de____________scription. Receipts missing even one piece of information cannot be used to apply for the subsidy.

  ・These are example receipts. Please ask your clinic to issue one receipt per recipient.

      

          

 5.Subsidy Payment Timing

  The subsidy will be paid to each employee’s salary payment account one month or two months after the  application month.

  The voluntary continued insureds will receive the subsidy at each designated back account.

 

 6.Others

  ❖ Those who receive TPC’s on-site flu shot program don’t need to apply for the subsidy.

  ❖Dependent family members under 13 can enter 2 flu shots information and upload both receipts in one application form.

  ❖Flu shots received overseas are not eligible.

  ❖Flu vaccines not approved in Japan (FluMist, etc.) are not eligible.

  ❖Only the cost of flu shot will be subsidized. Consultation fee, etc. will not be covered even if it is listed in the de____________scription section.
