Takeda Health Insurance Society starts a new initiative (Healthcare Subsidy Program) that helps “promote employee’s health”.
Whether you are already making efforts to improve your health or have not yet started, why not think about something you have always wanted to try to improve your health and give it a try by taking advantage of “health investment subsidy" provided by Takeda Kenpo based on “health investment”.
◇Eligible criteria：Takeda Kenpo Insureds during the eligible period + the voluntary continued insureds*Note that this program covers goods and services purchased by the insured person and/or any of his/her dependents (20 yrs of age of older), but ONLY the insured person can apply for the subsidy.
◇Subsidy limit per employee ：up to 30,000 for purchases (tax included)
◇Eligible period ：Between Friday, April 15 and Thursday, June 30, 2022
※Receipts dated during the above period are valid.
◇Application period ：Between Monday, May 9 and Thursday, June 30, 2022
◇Eligible items ：Purchases to improve employee’s health in the following
Sporting goods, sportswear, sports shoes, training equipment,
health products such as blood pressure monitors, activity meters, etc., admission and usage fees for FITNESS,
health foods, health drinks, and other items suitable to improve health

※Obtain Log in ID and PW at the initial page (make sure to prepare your health insurance card).
◆Note on application
・Apply for all purchases in one application. (Multiple applications are not acceptable.)
・Applicants are asked to enter what type of effort (health investment) they would like to make
・The user of the eligible items above include the insured and/or the adult dependent
・Applicants need to upload the digital image of shopping receipt(s) for their evidence
(If applicants have multiple receipts, upload them in one photo as a file)
[Important notice]Uploaded receipts should have the following information
① The Full name of the insured
② Actual cost (Note that the maximum amount of subsidy is 30,000 yen in total per applicant, and we disregard the excess amount over 30,000 yen. )
③ Itemized information (e.g. (as payment for something)
④ Date of issue (Note that the date should be within the aforementioned eligible period)
◇Payment of subsidy
・Bank account to be transferred: the insured receives the subsidy at a bank account designated for payroll and the voluntary continued insured receives at their designated bank (For Takeda employees, "Health Insurance Benefit" appears on applicant’s paycheck record)
・Transfer date: In principle, around the 25th of the second month following the month of application
(A crimped postcard notifying "Notice of Medical Expenses and Benefit Payment Amount” will arrive to the address of the applicant around the end of the month of transfer)
