武田健保では、健診受診率の低い被扶養者の健診受診を進めるため、毎年3月に総合健診案内をお送りしています。
今年も、2023年度の被扶養者向け総合健診案内を、被保険者のご自宅および単身赴任者の留守宅へ送付しました。
ぜひ、ご家族のみなさまへ健診受診を勧めていただきますよう、よろしくお願いいたします。
※冊子は日本語のみとなっています。
総合健診案内は、健保HPの「バックナンバー」からもご覧いただけます。
人間ドックをご利用希望の場合や、最新の健診機関情報は、WEB（KENPOS）でご確認ください。
※被保険者（本人）は、原則、会社の健診を受けてください。
予約期間：2023年 3月15日 ～ 2023年 9月30日（「受診券の発行依頼まで完了」厳守！）
受診期間：2023年 4月 1日 ～ 2024年 3月31日
【注意事項】
※受診日に武田健保の資格がない場合は受診できません。資格喪失後に受診された場合は、後日、健診費用をご返金いただきます。
※2023年9月30日までに予約が完了（受診券の発行依頼まで完了）している必要があります。
※2023年10月1日以降は、健診の予約はできません。（KENPOSでの健診機関の検索も出来ません。）

Takeda Health Insurance Society mails a booklet on health examination and checkups to each of the insureds in order to
raise the number of the dependents receiving health checkups whose health checkup coverage has been low every March.
We have sent the FY2023 booklet to each of the insured home address, and their family address for workers working away
from their families.
Now we ask each of you to encourage your family to take annual health checkups.

*The insureds are required to undergo the health checkups provided by Takeda.
Application acceptance period: Between March 15, 2023, and September 30, 2023 (Be sure to complete “Checkup Ticket Issuance Request”)
Checkup period: Between April 1, 2023, and March 31, 2024
[Important notes]
*Those who are disqualified from us on the consultation day cannot receive the health checkups provided by us. If you are no longer the insured and receive the health checkup provided by the Takeda Health Insurance Society, you should cover the costs for the annual health checkups later.
*Reservations must be made (complete Checkup Ticket Issuance Request) no later than September 30, 2023.
*Reservations are not available on and October 1, 2023. (You cannot search an in-network medical facility for health checkup at KENPO from October 1)
