Documents to submit when applying for dependent certification

Applying to name a child as a dependent (newborn, student)

To apply, provide the information specified on the following document and attach the specified certifying documents. Enclose a self-addressed return envelope if you are a Takeda Pharmaceutical Company Limited employee or a Voluntarily and Continuously Insured Person and submit to the Health Insurance Society. If you are not a Takeda Company Limited employee, submit to the section at your company responsible for health insurance administration.
Note that you may be asked to submit documentation in addition to the documents indicated below.

Certifying documents to attach

For a newborn child
◎Attach one of the following documents to verify the child’s name, date of birth, and gender and the names of both parents:
  • Copy of page certifying submission of birth notice in maternal and child health handbook
  • Certificate of residence (indicating all residents of household, all family members, and relationship between all residents)
◎In principle, if both parents work, the child should be the dependent of the parent whose income is higher. To allow income verification, attach certificates of income and withholding tax for both parents.
  1. Note: The date of dependent certification will be the child’s date of birth.
Other child (student)
  • Certificate of residence (indicating all residents of household, all family members, and relationship between all residents)
  • Certificate of school attendance if aged 18 or older and attending a university or college, preparatory school, or specialized school (if attending a performing arts school where a supplemental source of income may be expected, you may be asked to attach an income certificate as well)
  • Attach an official copy of your family register for cases involving adoption, divorce, or death of a spouse.
  • In principle, if both parents work, the child should be the dependent of the parent whose income is higher. To allow income verification, attach certificates of income and withholding tax for both parents.
    1. Note: Submit the above documents within five working days of the date on which the reason for dependency arose (including that date). If the Health Insurance Society receives the documents within five days and certifies dependency, certification will be retroactive to the date on which the reason for dependency arose.
      If the documents are received more than five working days later, the date of certification will be the date on which the Health Insurance Society received the documents and certified dependency.
