Is it necessary to apply to receive High-Cost Medical Care?

Application is unnecessary. Health Insurance Society calculates benefits based on "detailed medical care fee statements" provided by hospitals. Benefits are automatically deposited in bank accounts along with salaries.

For persons aged under 70, by presenting your Certificate of Application of Maximum copayment amount together with your insurance card at the reception desk of the hospital, you can ensure that the amount you pay at each medical institution will not exceed your individual cost-sharing maximum amount. To receive a Certificate of Application of Maximum copayment amount you need to apply to the Health Insurance Society in advance.
*When you use a Myna health insurance card, you will be exempt from payment in excess of the maximum copayment amount under the High-Cost Medical Care Benefits system without any prior procedures.
 We recommend using a Myna health insurance card. Using this card will eliminate the need to apply in advance for a Certificate of Application of Maximum Copayment Amount.
*For persons aged 70 or over, by presenting a medical care certificate for the elderly you can get the same treatment.
