 Please be advised that health insurance notification documents output from COMPANY were abolished and changed into new documents available at the Takeda Health Insurance Society as of April 1, 2022. The documents covered are as follows.

・Key things to note

①    Applicants who applied before the end of March should submit the application format output from COMPANY along with the required documents.

 

②  When applying for a new health insurance card, be sure to attach a return envelope with information on where to send the health insurance card (section name, etc.) filled out.
