Adding/removing a family member

Health insurance provides insurance benefits not just to insured persons but also to their dependent family members. These family members are referred to as “dependents”. Family members must meet certain conditions related to “residency in Japan,” “the extent of the family relationship,” and “income” before they are authorized as dependents.

Adding a family member

Required documents:
  • Application Form for Dependent Certification
Documents to attach for dependent certification("Refer to Documents to submit when applying for dependent certification")
  1. Note: Attach a preaddressed return envelope if you are a Takeda Pharmaceutical Company Limited employee or a Voluntarily and Continuously Insured Person.
Deadline:
Within five days after the reason for adding the family member arises
  1. Note: Submit the above documents within five working days of the date on which the reason for dependency arose (including that date). If the Health Insurance Society receives the documents within five days and certifies dependency, certification will be retroactive to the date on which the reason for dependency arose.
    If the documents are received more than five working days later, the date of certification will be the date on which the Health Insurance Society received the documents and certified dependency. Contact the Society in advance if you expect delays.
    (For a newborn child, the date of certification will be the date of birth.)
Applies to:
Insured persons for whom the number of dependent family members increases due to marriage, childbirth, etc.
Submit to:
Submit to the Health Insurance Society if you are a Takeda Pharmaceutical Company Limited employee or a Voluntarily and Continuously Insured Person. Submit to your employer’s person in charge of health insurance administration if you are an employee of an establishment other than Takeda Pharmaceutical Company Limited.

Removing a family member

Required documents:
  • Notification of Health Insurance Dependent (Change)
Health insurance card (for the dependent to be removed)
Elderly benefits card (if issued)
Deadline:
Within five days after the reason for adding the family member arises
Applies to:
[Insured persons with dependents who meet the following descriptions]
  • No longer eligible as dependents due to reasons such as employment, living apart from the insured person, or death
  • No longer satisfy the conditions for certification as dependents due to increased income
  • No longer rely on the income of the insured person for their livelihood because the insured person no longer provides an allowance
Submit to:
Submit to the Health Insurance Society if you are a Takeda Pharmaceutical Company Limited employee or a Voluntarily and Continuously Insured Person. Submit to your employer’s person in charge of health insurance administration if you are an employee of an establishment other than Takeda Pharmaceutical Company Limited.
