Marriage/divorce (change of name or address)
Apply to the Health Insurance Society if the name of the insured person or a dependent has changed or you want to add a new family member as a dependent due to marriage or other reason.

Change of name
To apply, provide the information specified on the following document and attach the health insurance card. Enclose a return envelope if you are a Takeda Pharmaceutical Company Limited employee or a Voluntarily and Continuously Insured Person, and submit to the Health Insurance Society. If you are an employee of an establishment other than Takeda Pharmaceutical Company Limited, submit to person in charge of health insurance administration at your employer.

- Takeda Pharmaceutical employees: Apply from Workday.
- Employees of other companies: Contact the person in charge of health insurance administration at your company.
