武田薬品健康保険組合で2025年度に実施する健康診断の案内冊子『2025年度 健康診断のご案内』を、3月中旬に被保険者様のご自宅および単身赴任者の現住所に郵送いたしましたので、ご確認いただきますようお願いいたします。
※ 2025年2月以降に入社された被保険者様については案内冊子を郵送しておりませんので、健保ホームページのお知らせ、「一般健診」「日帰りドック」の各ページでご確認をお願いいたします。
※ 『2025年度 健康診断のご案内』は日本語のみとなっています。
※ 被保険者（本人）様は、原則、会社の健診を受けてください。
※ 被扶養配偶者様向けに、2025年度限定の受診特典があります！！
2024年度に健康診断を受診した被扶養配偶者様が、2025年度も健康診断を受診された場合、2025年度分として「10,000ポイント」をプレゼントします（1ポイント＝1円相当）。
１． 健診コース（下記の2種類）
・ 一般健診
・ 日帰りドック
２． 予約期間と受診期間
・ 予約期間：2025年3月24日～2025年12月26日
（2025年12月27日以降、健診の予約はできません。）
・ 受診期間：2025年4月 7日～2026年 3月31日
３． 申込方法（下記のいずれか）

・ 電話予約：健診予約センター（0570-783-186）（健診機関への電話予約は不要）
４． 注意事項
・ 受診日に武田健保の資格がない場合は受診できません。武田健保を退会した後に受診された場合は、後日、健診費用をご返金いただきます。
・ 健診の無断キャンセルは医療機関に多大な迷惑がかかりますので、必ず予約センターに連絡してください。無断キャンセルの場合、キャンセル料をご負担いただく場合があります。また、体調不良等で当日やむを得ず健診が受診できない場合は、予約センターに加えて医療機関にもご連絡ください（健診を当日にキャンセルした、または、オプション検査を当日に受診できなかった場合も、キャンセル料を負担いただく場合があります）。
５． その他ご案内
・ 「特定健康診査受診券」について
40歳以上の被扶養家族・任意継続者（本人）・任意継続被扶養家族の方は、法定の特定健康診査の受診対象者となり、「一般健診」「日帰りドック」以外に、武田薬品健保が発行する「特定健康診査受診券」を使って健診を受診いただくこともできます。
・ 自治体がん検診について
「肺がん、胃がん、大腸がん、乳がん、子宮頸がん」のがん検診については、ほとんどの自治体で実施しており、一部の自己負担または無料でがん検診を受けることが出来ます。自治体からのがん検診のご案内を受け取った方は、自治体で実施するがん検診もご検討ください（自治体のがん検診を受診する場合、当健保のがん検診の受診は不要です）。
We will send a booklet titled "Health Checkup Guide in FY 2025" about a health checkup offered by the Takeda Pharmaceutical Health Insurance Society in FY 2025 to the insured’s home address and the current address of employees transferred without family by post in mid-March. Please check the contents.
* We do not send the booklet to the insured who joined the company in and after February 2025. So, please check the pages of "General health checkup" and "One-day complete medical checkup" in the notice on the Health Insurance website.
* “Health Checkup Guide in FY 2025” is available only in Japanese.
* In principle, the insured (employees) should take a health checkup of the company.
* Special offer limited to FY 2025 for dependent spouses who take a health checkup!!
When a dependent spouse who took a health checkup in FY 2024 takes a health checkup in FY 2025, 10,000 points will be given for FY 2025 as a gift (1 point =1 yen).
１．Health checkup course (the following 2 types)

２．Reservation period and visiting period
・Reservation period: Mar. 24, 2025 - Dec. 26, 2025
(You cannot make a reservation for a health checkup on and after December 27, 2025.)
・Visiting period: Apr. 7, 2025 - Mar. 31, 2026
３．Application method (either one of the following)

・Reservation by telephone: Health Checkup Reservation Center (0570-783-186) (It is not necessary to make a reservation at health checkup institutions by telephone.)
４．Precautions
・If you are not a member of the Takeda Pharmaceutical Health Insurance Society on the day of visit, you cannot take a health checkup. If you visit the institution after you withdrew from the Takeda Pharmaceutical Health Insurance Society, you will be asked to return the health checkup expense at a later date.
・Cancellation without notice will cause a lot of trouble to medical institutions. So, be sure to contact the Reservation Center. If you cancel without notice, you may need to pay the cancellation fee. If you cannot take a health checkup due to an unavoidable reason on the day such as poor physical condition, please contact the medical institution in addition to the Reservation Center (If you cancel a health checkup on the day or cannot undergo an optional test on the day, you may be asked to pay a cancellation fee).
５．Other information
・Specific Health Checkup Ticket
Dependents, insured persons with optional and continued coverage (employees), and dependents with optional and continued coverage (all aged 40 years or older) are the target of legal specific health checkup, and a health checkup can be taken with "Specific Health Checkup Ticket" issued by the Takeda Pharmaceutical Health Insurance Society other than "General health checkup" and "One-day complete health checkup."
・Local government's cancer screening
Cancer screening for "lung cancer, gastric cancer, colorectal cancer, breast cancer, and cervical cancer" is offered by most local governments and you can undergo a cancer screening with partial co-payment or free of charge. If you have received a guidance on cancer screening from the local government, please consider the cancer screening offered by the local government (If you undergo a cancer screening of the local government, you do not need to undergo a cancer screening of the Health Insurance Society).
