When you leave your employer

You will lose your eligibility for membership in the Health Insurance Society after you leave your employer. Join the appropriate medical care insurance system based on your needs and circumstances.

Return your health insurance card when you leave your employer

Required documents:
  • Health insurance cards (for the insured person and all dependents)
  • Elderly benefits card (if issued)
Deadline: Within five days after the date of loss of eligibility
Applies to: Insured persons leaving employment and their dependents
Address inquiries to: Health Insurance Society
Notes:  

If you wish to remain a member of the Health Insurance Society

Required documents:
  • Application Form for Certification as Voluntarily and Continuously Insured Person
Deadline: Within 20 days after the date of loss of eligibility of the insured person
Applies to: Insured persons who have been insured continuously for at least two months prior to leaving employment
Address inquiries to: Health Insurance Society
Required documents:
    • I. For dependents certified while you were employed, you need only fill out the Dependent Notification section.
    • II. To add a new dependent while you are a Voluntarily and Continuously Insured Person, submit the Application Form for Dependent Certification.
Document to attach: In case I above:
No documents need to be attached for dependent certification.
(You may be asked to submit documents if the Society needs to verify certain information.)

In case II above:
Documents must be attached for dependent certification(Refer to “Documents to submit when applying for dependent certification”)
