Death

The Health Insurance Society will pay “Funeral Expenses” in the event of the death of an insured person or dependent. It will pay “Funeral Costs” to the person who actually administered the burial of a deceased person in the event of a deceased person with no family members or close friends.

If the insured person has died

Required documents:
  • Claim for Funeral Expenses/Funeral Costs (Insured Person, Dependent)
  • Documents certifying the death
    (copies of Death Certificate and Burial Permit or Cremation Permit)
Deadline: As soon as possible
Applies to:
  • Surviving dependents (Funeral Expenses)
  • The person who actually handled the burial of a deceased person if the deceased person had no family members or close friends (Funeral Costs)
Address inquiries to: Health Insurance Society
Notes: When requesting payment of Funeral Costs, attach receipts indicating the cost of the burial, in addition to the Death Certificate.
To apply, provide the necessary information on the above document, attach the documents to attach, and submit to the Health Insurance Society if you are a Voluntarily and Continuously Insured Person. If you are not a Voluntarily and Continuously Insured Person, submit to the section at your company responsible for health insurance administration.

If a family member has died

Required documents:
  • Claim for Funeral Expenses/Funeral Costs
  • Documents certifying the death
    (copies of Death Certificate and Burial Permit or Cremation Permit)
Deadline: As soon as possible
Applies to: Person on whom the deceased was dependent

Notes: To apply, provide the necessary information on the above document, attach the documents to attach, and submit to the Health Insurance Society if you are a Takeda Pharmaceutical employee or a Voluntarily and Continuously Insured Person. If you are not a Takeda Pharmaceutical employee or a Voluntarily and Continuously Insured Person, submit to the section at your company responsible for health insurance administration. You will also need to complete procedures to remove the deceased as a dependent. See here for details.
