Death
The Health Insurance Society will pay “Funeral Expenses” in the event of the death of an insured person or dependent. It will pay “Funeral Costs” to the person who actually administered the burial of a deceased person in the event of a deceased person with no family members or close friends.

Notes: | When requesting payment of Funeral Costs, attach receipts indicating the cost of the burial, in addition to the Death Certificate. To apply, provide the necessary information on the above document, attach the documents to attach, and submit to the Health Insurance Society if you are a Voluntarily and Continuously Insured Person. If you are not a Voluntarily and Continuously Insured Person, submit to the section at your company responsible for health insurance administration. |

To apply, provide the necessary information on the above document, attach the documents to attach, and submit to the Health Insurance Society if you are a Takeda Pharmaceutical employee or a Voluntarily and Continuously Insured Person. If you are not a Takeda Pharmaceutical employee or a Voluntarily and Continuously Insured Person, submit to the section at your company responsible for health insurance administration.
