Childbirth

“A Childbirth and Childcare Lump-Sum Grant” is paid when an insured person gives birth. “A Dependents' Childbirth and Childcare Lump-Sum Grant” is paid when a dependent family member gives birth. Don't forget to add the newborn as a dependent in these cases.

Request a Childbirth and Childcare Lump-Sum Grant

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Society. For details, check with the medical care institution or other facility where you plan to give birth.)

When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Society in advance.

Required documents:
  • Claim for Childbirth and Childcare Lump-sum Grant (Insured Person, Dependent)
Deadline: In advance
Applies to: Insured persons and dependents expecting childbirth up to two months in the future who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf
Address inquiries to: Health Insurance Society
Notes:  

If you paid the entire amount of childbirth expenses to the medical care institution

Submit the following application to the Health Insurance Society if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf or you have given birth overseas.

Required documents:
  • Claim for Childbirth and Childcare Lump-sum Grant (Insured Person, Dependent)

  • Copies of agreements from the medical care institution or other facility
    (documents demonstrating that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and indicating the name of the Health Insurance Society as application recipient)
  • Obtain a certificate from the doctor (or midwife) or a certificate of submission of birth notice (copies not accepted) or abstract of your family register (copies not accepted) issued by the mayor of your municipality and submit to the Health Insurance Society if you are a Takeda Pharmaceutical Company Limited employee. If you are not a Takeda Pharmaceutical Company Limited employee, submit to the section at your company responsible for health insurance administration.
  • Copies of receipts for and details of childbirth expenses
    (documents demonstrating that the details are the same as those on the special invoice submitted by the medical care institution or other facility to the Health Insurance Society and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)
Deadline: As soon as possible
Applies to: Insured persons or dependents who have paid the entire amount of childbirth expense at the medical care institution without using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

Notes: Although the above documents to attach (copies of agreements and copies of receipts for and details of childbirth expenses) are not required for childbirth overseas, if no certification is provided in the doctor’s certification space on the Claim for Childbirth and Childcare Lump-sum Grant, attach documentation such as an (original) birth certificate from the medical care institution or national (or municipal) government or an (original) abstract of the family register.
Also submit the following documents if you are claiming only the Childbirth and Childcare Lump-sum Grant and not applying for dependent certification for the newborn child.
  • ① Copies of documentation of overseas travel (e.g., passport, boarding tickets)
  • ② A letter of consent allowing the Society to confirm the fact and details of childbirth with the overseas medical care institution

  • * The documents under ① and ② above are not required from those on overseas job assignments (and their accompanying family members).
Notes on filling out the form
  • Apply only if you were unable to use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or if the child was born overseas.
  • When applying, affix the same seals as the applicant’s official seal and the seal used for proxy receipt.
  • Applications without a certificate from the doctor (or midwife) or a certificate issued by the mayor of the municipality attached cannot be accepted. (Copies of the maternal and child health handbook are not acceptable.)

Add the child

After the child is born, take the necessary steps to add him or her as a dependent.
