“A Childbirth and Childcare Lump-Sum Grant” is paid when an insured person gives birth. “A Dependents' Childbirth and Childcare Lump-Sum Grant” is paid when a dependent family member gives birth. Don't forget to add the newborn as a dependent in these cases.

Request a Childbirth and Childcare Lump-Sum Grant
When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions
Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Society. For details, check with the medical care institution or other facility where you plan to give birth.)
When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf
If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Society in advance.

If you paid the entire amount of childbirth expenses to the medical care institution
Submit the following application to the Health Insurance Society if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf or you have given birth overseas.

Notes: | Although the above documents to attach (copies of agreements and copies of receipts for and details of childbirth expenses) are not required for childbirth overseas, if no certification is provided in the doctor’s certification space on the Claim for Childbirth and Childcare Lump-sum Grant, attach documentation such as an (original) birth certificate from the medical care institution or national (or municipal) government or an (original) abstract of the family register. Also submit the following documents if you are claiming only the Childbirth and Childcare Lump-sum Grant and not applying for dependent certification for the newborn child.
