If an insured person is unable to work to undergo treatment for a nonoccupational sickness or injury and is unable to earn salary or other income during this time, he or she will be paid “Injury and Sickness Allowance”.

Notes: | You must satisfy all four of the following conditions to receive this allowance:
Provide the necessary information on the above document. If you are a Takeda Pharmaceutical Company Limited employee, send it to the HR Form Submission Point of Contact, Osaka HQ. Send the documents for procedures after loss of eligibility as an insured person directly to the Health Insurance Society. If you are not a Takeda Pharmaceutical Company Limited employee, submit to the section at your company responsible for health insurance administration. |
